What Causes Secondary Dysmenorrhea (Period Pain)?

Secondary dysmenorrhea—period pain—is often a sign of underlying gynecologic disease. Dr. Karli Goldstein breaks down the leading causes, from endometriosis to fibroids and uterine anomalies.

A Closer Look at Painful Periods Beyond Day One

In this expert short, Dr. Karli Provost Goldstein walks through the underlying causes of secondary dysmenorrhea—a clinical term for painful periods that develop after previously pain-free cycles. Unlike primary dysmenorrhea, which begins with the first period (menarche), secondary dysmenorrhea signals an evolving or new source of pain and should prompt evaluation.

Dr. Goldstein outlines several causes behind this delayed-onset period pain:

  • Endometriosis is one of the most common causes, affecting 30–80% of patients with pelvic pain. It can begin in adolescence or emerge in adulthood (20s through 40s), even after childbirth. Pain may start one to two days before bleeding begins, continue throughout menstruation, or occur at other points in the cycle—not just during the period itself.
  • Adenomyosis, a condition where endometrial tissue infiltrates the muscular wall of the uterus, is another common cause. Like endometriosis, it can appear at any age and often worsens over time.
  • Uterine fibroids, particularly those inside the uterine cavity or pressing on adjacent organs, may also cause pain. However, Dr. Goldstein notes that many large fibroids, especially intramural ones, cause bulk symptoms like pressure and heavy bleeding without significant pain—which can delay diagnosis and treatment.
  • Congenital uterine anomalies, such as a bicornuate or unicornuate uterus, as well as uterine septums, can obstruct menstrual flow and lead to cramping. Additional causes include infections of the uterus, tubes, or ovaries, and adhesions from childbirth or surgery.

The key insight is that secondary dysmenorrhea isn’t just about period day pain—it can reflect chronic pelvic conditions that require a precise, individualized approach. When symptoms shift or worsen, it’s time to take a closer look.
