During hysteroscopy, a large endometrial polyp was identified within the uterine cavity. Polyps are a common yet frequently missed source of abnormal uterine bleeding and pelvic pain, particularly when imaging appears normal. They are often hormonally driven and are seen more commonly in patients with endometriosis, where estrogen and chronic inflammation play a key role. The remainder of the uterine cavity, including the fallopian tube openings, appeared normal.
The polyp was removed during the same minimally invasive procedure, allowing for both diagnosis and treatment in one setting. This case highlights the importance of directly evaluating the uterine cavity when symptoms persist and reinforces the need to consider causes of pain, bleeding, and infertility both inside and outside the uterus.
