Every refill goes through clinical review and pharmacy processing. Use the guide below to know when to request yours, then submit the form.

When to Request Your Refill

Submitting on time helps us ensure your treatment continues without interruption.

  • Birth Control 1 week ahead
    Request when you start your last week of pills.
  • Hormone Replacement 1–2 weeks ahead
    Request when you begin your last tube, or open your last patch in the box.
  • GLP Medications 2–3 weeks ahead
    Request when you reach ¼ remaining in your vial. GLPs ship on ice Monday–Thursday only, which is why we ask for additional lead time.

Important to Know

Dosage changes require a visit. Any alteration to your dosing requires a phone call or appointment with our Nurse or PA, along with updated labs. Please do not request a dose change through this form.

Controlled substances require additional review. Refills for controlled substances must be processed with a DEA-credentialed provider and a triple-authenticator review, which adds time to the standard turnaround.
