Dr. Leigh Rosen and colleagues at the Icahn School of Medicine at Mount Sinai publish a randomized controlled trial in Anesthesiology evaluating oral olanzapine for the prevention of postdischarge nausea and vomiting after ambulatory gynecologic and plastic surgery. In this single-center, double-blind, placebo-controlled trial of 140 adult female patients, a single preoperative 10 mg dose of olanzapine — added to standard dexamethasone and ondansetron prophylaxis — reduced nausea and/or vomiting in the 24 hours after discharge from 38% in the placebo group to 14% in the olanzapine group (relative risk 0.37, P=0.003), a roughly 60% reduction, with only a modest increase in reported sedation.
