Provider Pricing

Office Visit Fees (Out-of-Network):

Office visits are charged upfront for the time required. You will receive a receipt and a super-bill to submit to your insurance for coverage. The super-bill includes specific insurance coding related to your diagnosis and visit, including exams and ultrasounds.

Please note: The cost of your visit will be applied to your out-of-network deductible. If met, your insurance may reimburse a portion of the visit. This will be sent directly to you by your insurance.

Dr. Karli Provost Goldstein is a board-certified, fellowship-trained expert in minimally invasive gynecologic surgery, specializing in advanced laparoscopic and robotic excision of endometriosis, fibroids, adenomyosis, and pelvic pain. She uses fertility-preserving techniques to treat even the most complex cases involving the bowel, bladder, diaphragm, and nerves, performing deep excision through tiny incisions for long-term relief and optimal recovery.

At ESSE Care and Wellness, Dr. Goldstein takes a whole-body, hormone-aware approach, combining expert surgical care with personalized treatment plans that address root causes of pain, inflammation, and hormonal imbalance. We are out of network with ALL major insurance providers. However, with CERTAIN PLANS, we can work with your insurance for coverage on the  surgical fees and associated costs, depending on your POLICY. In certain cases we are able to obtain a one-time  approval for  the surgery as an in network exception

Please note: Tricare, Medicare, and Medicaid do not reimburse physicians out of their network. These will be treated as cash payments.

 

Superbills & Insurance Coverage

For out-of-network services, we provide detailed superbills containing:

  • Specific diagnosis codes (ICD-10)
  • Procedure codes (CPT) for all services performed
  • Detailed receipt showing your payment

 

You can submit this superbill to your insurance company for possible coverage, which will be sent directly to you. The visit cost will also be applied toward your out-of-network deductible.

Note: Records exceeding 50 pages may require a consultation fee due to the extensive review time needed.
