Insurance Information
At ESSE Care we strive to work with your insurance provider for surgical costs. However, we do not bill providers for office visits and will provide you with a superbill for your visit.
We are happy to call your insurance on your behalf and provide a complimentary update on your plan and what you have met for deductibles and out-of-pocket costs.
By providing your basic information and insurance card we can have our team contact them and find out your benefits.
Esse Care Consult fee - $250 per 15 minutes required
$1,250 – 1 hour 15 minutes
$1,000 – 1 hour
Insurance Plans
We are out of network with most major insurance providers. However, we can work with your insurance for surgical fees and associated costs, depending on your plan. Many times we are even able to work with in network insurance plans for surgical fees if they agree to care from a specialist (approval case by case).

Our office and billing department can assist in verifying your benefits and potential costs. To do this, we require a copy of your insurance card (front and back) and some basic demographic information. This process ensures transparency about costs before your appointment.
Please note: Tricare, Medicare, and Medicaid do not reimburse physicians out of their network. These will be treated as cash payments.
We accept and encourage you to use flexible spending and health savings account debit cards for all services. These pre-tax accounts can help offset out-of-pocket costs.
Checking Your Insurance Coverage
To understand your out-of-network benefits, you can contact your insurance provider using the number on the back of your card and ask the following questions:
- Do I have out-of-network coverage for physician services, imaging, and surgery?
- What percentage of the visit is covered?
- Is there a limit to how many visits are covered per year?
- Do I have an out-of-network deductible to meet? What is my out-of-pocket maximum?
- What diagnoses are covered?
- How do I submit super-bills for reimbursement? (Please email us the required forms if necessary.)

Office Visits and Super-Bills
Office visits are charged upfront for the time required. You will receive a receipt and a super-bill to submit to your insurance for reimbursement. The super-bill includes specific insurance coding related to your diagnosis and visit, including exams and ultrasounds.
Please note: The cost of your visit will be applied to your out-of-network deductible. If met, your insurance may reimburse a portion of the visit. This will be sent directly to you by your insurance.
Surgical Fees
For surgical procedures, we will work extensively with your insurance for fee coverage. After we have reviewed the complexity of your case and your specific insurance plan with necessary coding and time required we can find out more about what your insurance plan covers.
Case Reviews
We offer case reviews to determine if you are a candidate for our practice. Please provide recent lab work (within 6 months), imaging (within the last year), and operative reports with photos and pathology. Charts over 50 pages will incur a full consult fee for the time required to review them.
